Yukar\u0131daki hususlar\u0131 do\u011fru bir \u015fekilde haz\u0131rlamaz ve uygulamazsan\u0131z, aksi takdirde testin muayene g\u00fcn\u00fcnde yap\u0131lamamas\u0131 ve yine de \u00fccretlendirilmesi gerekebilir. Daha sonra veri koruma onay formunu imzalamal\u0131 ve gerekirse sevk belgesini g\u00f6stermelisiniz.
